Job Description: Home Health Registered Nurse / Licensed Practical Nurse

Position Overview:

We are seeking a compassionate and experienced Registered Nurse to join our home health care team. As a Home Health Registered Nurse, you will play a critical role in delivering comprehensive and personalized care to patients in the comfort of their own homes. Your clinical expertise, assessment skills, and dedication to patient-centered care will contribute to improving the health and well-being of our clients.

Responsibilities:

1. Patient Assessment and Care Planning: Conduct thorough assessments of patients' medical conditions, functional abilities, and psychosocial needs within their home environment. Develop individualized care plans based on assessment findings, collaborating with interdisciplinary team members to ensure holistic and goal-oriented care.

2. Direct Patient Care: Provide skilled nursing care, including wound care, medication administration, IV therapy, pain management, and other treatments as prescribed. Monitor and evaluate patients' health status, document observations, and report any significant changes to the physician and care team.

3. Patient Education: Educate patients and their families about their conditions, treatment plans, and self-care techniques. Provide guidance on medication management, symptom management, dietary modifications, and other aspects of care to promote optimal health and independence.

4. Coordination and Collaboration: Collaborate with physicians, therapists, social workers, and other healthcare professionals to coordinate patient care and ensure continuity of services. Communicate effectively with the interdisciplinary team, patients, and their families to foster a collaborative and patient-centered care approach.

5. Documentation and Compliance: Maintain accurate and up-to-date patient records, including assessments, care plans, progress notes, and other relevant documentation in compliance with regulatory standards and agency policies. Adhere to HIPAA regulations and maintain strict confidentiality of patient information.

6. Home Safety Evaluation: Assess patients' living environments for safety hazards and provide recommendations to optimize safety and prevent falls or other accidents. Educate patients and families on home safety measures and assistive devices to promote a secure and supportive living environment.

7. Quality Assurance: Participate in quality improvement initiatives by actively identifying areas for improvement, sharing best practices, and contributing to the development and implementation of evidence-based care protocols. Adhere to established quality standards and regulatory guidelines specific to home health care.

8. Professional Development: Stay updated with advancements in home health care, evidence-based practices, and relevant regulations through continuing education and professional development activities. Share knowledge and mentor other team members to promote a culture of continuous learning.

Qualifications:

- Valid Registered Nurse (RN) license and relevant nursing degree from an accredited institution.

- Minimum of 2 of nursing experience, preferably in home health care or a related field.

- Strong clinical assessment and critical thinking skills to provide comprehensive care within a home care setting.

- Proficient in conducting patient assessments, developing care plans, and delivering skilled nursing interventions.

- Excellent communication and interpersonal skills to effectively interact with patients, families, and the interdisciplinary care team.

- Knowledge of home health care regulations, compliance standards, and best practices.

- Ability to work autonomously, prioritize tasks, and manage time effectively while maintaining a high level of attention to detail.

- Compassionate, empathetic, and patient-centered approach to care.

- Familiarity with electronic health record systems and proficiency in documentation.

- Current CPR certification.

- Valid driver's license and reliable transportation for home visits.

Required Skills:
·Excellent written and verbal communication skills with the ability to explain complex medical terminology clearly and concisely.
·Strong organizational skills with the ability to perform a variety of routine office tasks as well as complex computer operations.
·Ability to maintain confidentiality of sensitive information. Ability to handle sensitive situations that may arise in the office or clinical environment. Ability to work effectively with a variety of personalities in an environment that is dynamic, changing, and requires flexibility as needed.
·Ability to work effectively under pressure in a fast-paced environment while maintaining composure under stress. Ability to work effectively with others in a team environment where collaboration is critical. Strong computer skills including Microsoft Office Suite (Word, Excel, PowerPoint) is required along with knowledge of basic web search tools such as Google Google Docs/Drive. Knowledge of EMR (Excel) is required along with knowledge of basic EMR/EMR systems such as LifePoint or LifeBridge is preferred but not required.
